Qualifications
- Active Nurse Practitioner license
- National board certification
- Current state licensure and DEA registration (as applicable)
Responsibilities
- Provide pain and symptom management in outpatient palliative care
- Lead goals-of-care and advance care planning discussions
- Support hospice readiness and referrals when appropriate
- Collaborate with physicians, home health, hospice, and referral partners
- Participate in interdisciplinary team meetings and care conferences
- Coordinate transitions across outpatient, hospital, home health, and hospice settings
- Serve as a clinical resource and patient advocate
