Job summary

Ennoble Care is seeking an experienced Palliative Care Nurse Practitioner for Atlantic County, NJ. The role focuses on compassionate outpatient palliative medicine, symptom management, and care coordination for patients with serious illness.

You will lead goals-of-care conversations, support families through complex journeys, and collaborate with a multidisciplinary team to ensure dignity and comfort throughout transitions across care settings.

Qualifications

• NP with board certification in palliative care or related specialty.
• Active state NP license and DEA registration as applicable.
• Experience in outpatient palliative care and complex symptom management preferred.

Responsibilities

• Provide pain and symptom management in the outpatient setting.
• Lead goals-of-care discussions and advance care planning.
• Support hospice readiness and coordinate referrals when appropriate.
• Collaborate with physicians, home health, hospice teams, and community partners.
• Participate in interdisciplinary team meetings and care conferences.
