Qualifications

• Doctoral degree in Physical Therapy from an accredited school.
• Current CPR certification required in Illinois.
• Strong interpersonal and communication skills.
• Ability to lift 20–50 pounds occasionally and 10–25 pounds frequently.

Responsibilities

• Evaluate patients’ physical and functional status and adjust therapy accordingly.
• Establish therapy goals and formulate treatment plans with activities and modalities.
• Discuss evaluation results and exercise program with patients and families; instruct use of equipment.
• Document evaluations and progress for physicians and insurance carriers.
