Qualifications

• Licensed Occupational Therapist (OT) – Illinois IDFPR.
• Bachelor’s Degree: Occupational Therapy (Required).
• BLS within 30 days - American Heart Association (AHA).

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed and ensures safe body mechanics.
• Educates patients, families and caregivers and collaborates with the rehabilitation team on goals and discharge plan.
• Documents evaluations and plans of care in accordance with department policy and regulatory standards.
