Job summary

Qualifications

• BLS within 30 days (AHA)
• Licensed OT in the appropriate state
• Bachelor’s Degree: Occupational Therapy required

Responsibilities

• Transfers patients with assistance as needed from bed, chair or floor
• Maintain safe body mechanics to protect patient and staff
• Design and implement a care plan with goals, treatment, outcomes, and discharge plan
• Modify care based on patient response and evidence
• Educate patients, families, and caregivers using effective teaching methods
• Evaluate findings to determine physical therapy diagnoses and outcomes
• Perform examinations using reliable measures relevant to chief complaint and history
• Screen patients to determine need for further therapy services
• Utilize support personnel per legal/ethical guidelines
• Monitor care delivered and provide feedback to staff
• Document evaluations and plans of care per policy, including goals and status
• Complete annual education to comply with program requirements
• Implement intervention plans focused on functional activities and patient needs
• Adapt approach as patient behavior and performance change
• Use standardized outcome measures to reflect functional performance
• Maintain compliance with regulatory and organizational documentation
• Collaborate with patient and family throughout rehab and provide training as needed
• Supervise COTA and rehab technicians per regulations
• Clearly communicate patient status and discharge needs to the medical team
