Job summary

Beloit Health System is seeking an Occupational Therapist to join our outpatient rehabilitation team at NorthPointe Clinic in Illinois. You will evaluate, develop, and implement individualized treatment plans to help patients improve functional abilities, independence, and quality of life.

Qualifications

• Bachelor or Master's in Occupational Therapy or Certificate degree from an accredited school or higher degree-equivalent.
• Satisfactory completion of a 6-9 month internship program in Occupational Therapy.
• Current Occupational Therapy License in Illinois.
• CPR Certification within 3 months of hire.

Responsibilities

• Evaluate patients' physical, cognitive, sensory, and psychosocial functioning.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Provide therapeutic interventions to improve daily living skills, upper extremity function, coordination, strength, and cognitive performance.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and adaptive strategies.
• Recommend adaptive equipment, assistive devices, and environmental modifications to promote independence.
• Accurately document evaluations, treatment progress, goals, and discharge plans in the electronic medical record.
• Collaborate with physicians, physical therapists, speech therapists, and other healthcare professionals to provide comprehensive patient care.
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
• Maintain compliance with regulatory requirements, clinic policies, and professional standards of practice.
• Participate in quality improvement initiatives and professional development activities
