First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant for our primary care practice in Manchester, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting.

Standard Job Duties:

• Evaluates, assesses patient illness/injuries.
• Performs physical examinations, and special examinations.
• Takes medical histories.
• Develops plans of care.
• Provides routine and illness care.
• Orders medications; orders diagnostic tests; and evaluates results.
• Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
• Maintains patient medical records.
• Participates in medical student and health professional teaching.
• Performs related duties as required.
