Qualifications

• Proven experience as a Nurse Practitioner or Physician Assistant establishing care plans and managing patient outcomes.
• A current license to practice as a NP or PA in the state of Connecticut.

• Excellent communication, interpersonal, and organizational skills.
• Connecticut Controlled Substance license and DEA license.
• BLS Certification through the American Heart Association or American Red Cross.

Responsibilities

• Evaluates and manages patient care in an outpatient setting.
• Develops plans of care and coordinates follow-up and referrals.
• Orders medications and diagnostic tests and evaluates results.
• Maintains patient medical records and participates in teaching of students and staff.
• Performs related duties as required.
