JOB DESCRIPTION
POSITION TITLE:
Primary Care Nurse Practitioner / Physician Assistant
POSITION SUMMARY:
The Primary Care Nurse Practitioner / Physician Assistant (NP/PA) is responsible for providing comprehensive, patient-centered medical care to individuals and families across the lifespan, with a focus on prevention, chronic disease management, and health equity. The role includes diagnosing and treating acute and chronic conditions, performing routine exams, coordinating care with interdisciplinary teams, and addressing social determinants of health that impact patient outcomes. NP/PAs are expected to deliver culturally responsive care, support access for underserved populations, and contribute to quality improvement initiatives while adhering to evidence-based practices and regulatory standards.

Key Responsibilities
Direct Patient Care Responsibilities
Maintains an active patient panel consistent with organizational access, panel size, and productivity expectations and provides the full scope of preventive and primary care services within professional licensure and scope of practice, including:
Quality Improvement & Compliance
Credentialing & Professional Standards
Maintains all required licensure, certifications, and credentials necessary for clinical practice and payer enrollment as well as:
Immediately notifies administration of any adverse clinical event, pending litigation, investigation, or disciplinary action by a licensing board.
