Universal Health Services, Inc. operates Calvary Healing Center's Phoenix campus seeking an Outpatient Navigator. This full-time day-shift role supports the Clinical Director by guiding patients from inpatient to outpatient services and by improving access and flow within outpatient programs.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree or Associate degree with at least 6 months of behavioral health experience preferred.
• Must be at least 21 years old; valid AZ Fingerprint Clearance Card or able to obtain within 30 days.
• Minimum of 2 years of experience in case management.
• Completion of BLS and First Aid within 30 days; Handle With Care training within 30 days.

Responsibilities

• Assists with outpatient program growth under Clinical Director guidance.
• Facilitates transition of patients from inpatient to outpatient programs to improve flow.
• Supports access to outpatient care and coordinates services.
• Contributes to a patient-centered, supportive care environment.

Position: Full-Time

Shift: Day

For over 60 years, Calvary Healing Center has provided a full continuum of care, specializing in addiction treatment for adults across the Phoenix area and Nationwide. Calvary’s proven treatment programs address the practical, physical, and spiritual aspects of addiction for lasting recovery. Our flagship location in Phoenix offers on-site detoxification and inpatient programs, a partial hospitalization program (PHP), and an intensive outpatient program (IOP). We also have satellite clinics offering Intensive Outpatient Programs in the West Valley region and Metropolitan Phoenix. Summary of Duties:

The Outpatient Navigator works under the guidance of the Clinical Director. The Outpatient Navigator assists with the overall internal growth of the outpatient programs. The Outpatient Navigator identifies, educates, and assists in facilitating individuals from the inpatient unit to the outpatient programs. The Outpatient Navigator also works to improve the overall patient flow and access to outpatient care.
