Job summary

Northeast Health Services, a member of Transformations Care Network, is seeking licensed mental health therapists to join a collaborative clinical team. You will provide outpatient individual and family therapy, develop safety and relapse plans, and participate in case consultations and discharge planning.

The role emphasizes culturally sensitive assessments, timely documentation, and teamwork with psychiatrists and other professionals to support client care and licensure growth.

Qualifications

• Master’s degree in Social Work, Counseling, Marriage and Family Counseling, or related field.
• Provisional license per state regulation may be required.
• Educational background leading to professional licensure.

Responsibilities

• Conduct culturally sensitive, strength-based assessments for new clients and families.
• Provide outpatient individual and family therapy tailored to each client.
• Develop crisis, safety, relapse, and wellness plans.
• Participate in case consultations, meetings, and discharge planning.
• Complete billing, documentation, and insurance authorization forms timely.
• Collaborate with psychiatrists, nurse practitioners, dietitians, and therapists.
