Job summary

INTEGRIS Health is seeking a Pharmacist Specialist in Medication Management for day shift, full-time work at our Oklahoma City location. The role focuses on comprehensive medication management, patient education, and collaboration with care teams to optimize therapy outcomes.

The ideal candidate will have PGY1 residency or equivalent, a degree in pharmacy, and licensure in Oklahoma or eligibility with a grace period.

Qualifications

• PGY1 residency or equivalent experience.
• Graduation from an accredited School of Pharmacy.
• Advanced degree or residency/fellowship or equivalent experience.
• Oklahoma Pharmacist License or eligible with grace period for licensure.

Responsibilities

• Provides high-quality, cost-effective pharmaceutical care to specific patient populations within the scope of practice.
• Adheres to National Patient Safety Goals and supports comprehensive medication management (CMM).
• Responds to drug information requests from healthcare professionals.
• Oversees formulary-approved medications and ensures formulary availability.
• Reconciles medication histories and assesses regimen efficacy, safety, interactions, and costs.

Education

Graduation from a program in Pharmacy from an accredited school of pharmacy

Advanced degree in pharmacy and/or a pharmacy residency or fellowship or equivalent experience
