Qualifications

Responsibilities

• Provide comprehensive outpatient primary care to patients.
• Diagnose and manage acute and chronic conditions.
• Develop and implement individualized treatment plans.
• Conduct follow-up visits and monitor patient progress.
• Utilize eClinicalWorks EMR for documentation.
• Collaborate with nurses, medical assistants, and other clinicians.
• Participate in quality improvement and patient education initiatives.
• Ensure culturally competent communication, including Spanish proficiency.
