Qualifikationen

• Experience in addiction recovery or behavioral health settings.
• Familiarity with MAT (methadone, buprenorphine, naltrexone).
• Experience with individual and group counseling for substance use recovery.
• Knowledge of CBT, MI, relapse prevention techniques.
• Documentation of clinical notes and treatment plans per guidelines.
• Familiarity with state regulations (e.g., 42 CFR Part 2, HIPAA).
• Experience collaborating in a multidisciplinary team.

Aufgaben

• Provide high quality, compassionate guidance in individual and group sessions.
• Plan, oversee, facilitate and document patient recovery.
• Co-facilitate group or family sessions as needed.
• Ensure documentation for care, treatment and incidents is timely and clear.
• Prepare individual treatment plans for assigned patients.
• Conduct initial and follow-up assessments.
• Evaluate patient needs and referrals to other programs as needed.
• Provide crisis intervention when required.
• Perform case management to ensure quality care.
• Act as liaison between referral sources and patients.
