Job summary

Concord Comprehensive Treatment Center in Concord, NH, is seeking a Substance Use Counselor for a full-time role in outpatient MAT services. Counselors provide compassionate individual and group sessions, develop treatment plans, and coordinate care to support recovery from opioid use disorder.

The position emphasizes early morning hours, collaboration with a multidisciplinary team, and opportunities for professional growth within Acadia Healthcare's network of centers.

Qualifications

• Associate's degree from an accredited college required.
• Bachelor's or Master's in social/health services preferred.
• Experience in addiction recovery or behavioral health settings.
• Knowledge of MAT medications such as methadone, buprenorphine, and naltrexone.
• Experience documenting clinical notes and treatment plans.

Responsibilities

• Provide high quality, compassionate individual and group counseling sessions.
• Plan, oversee, facilitate and document patient recovery.
• Co-facilitate assigned group or family sessions as needed.
• Ensure timely and clear documentation of patient care, treatment, and incidents.
• Prepare individualized treatment plans for each patient.
• Conduct initial and follow-up assessments.
• Evaluate patient needs and referrals to other programs or facilities.
• May plan for aftercare for assigned patients.
• Provide crisis intervention to patients as needed.
• Provide case management to ensure individualized quality care.
• Act as a liaison between referral sources and patients.
