Job summary

Community Health and Dental Care, Inc. is seeking a Licensed Practical Nurse for our outpatient primary care network. The LPN will provide direct patient care, support clinical workflows, and assist providers across CHDC sites.

Responsibilities include patient intake, vital signs, medication administration, and documentation in the EHR. Travel to CHDC sites is required to ensure coverage and continuity of care.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
• Current CPR certification or ability to obtain upon hire.

Responsibilities

• Prepare patients for provider visits by obtaining and documenting vital signs, medical history updates, medication reconciliation, and required screenings.
• Accurately document patient information and clinical findings in the electronic health record (EHR).
• Assist physicians, physician assistants, and nurse practitioners with examinations, procedures, and treatments.
• Administer medications, immunizations, injections, and other treatments as ordered and within the scope of licensure.
• Perform clinical procedures, including specimen collection, point-of-care testing, EKGs, wound care, and other approved nursing interventions.
• Escalate urgent clinical concerns according to established protocols and document all patient interactions in the EHR.
• Assist in coordinating referrals, diagnostic testing, patient outreach, prescription refill requests, and other care management activities.
