Job summary

Southeast Healthcare, LLC is seeking an experienced Licensed Practical Nurse (LPN) to coordinate and deliver nursing care in our outpatient clinic. The role involves administering injections, dispensing medications per orders, monitoring patient health, and educating patients to support ongoing care.

You will work closely with physicians, behavioral health staff, and other team members to ensure quality patient care, efficient clinic operations, and continuity of services.

Qualifications

• Current, valid LPN license in Ohio, in good standing.
• Strong communication, organization, documentation, and patient service skills.
• Ability to work collaboratively with multidisciplinary teams.
• Adherence to confidentiality and clinical procedures.

Responsibilities

• Administer injections and distribute medications per orders and protocols.
• Complete clinical documentation and maintain accurate patient records.
• Monitor vital signs, perform finger-stick tests, and other in-office services.
• Provide patient education and coordinate care with providers and community resources.
• Schedule appointments, manage referrals, reminders, and nursing line inquiries.
• Maintain medication inventory and ensure proper storage and disposal.
