Job summary

Appalachian Mountain Health is seeking an experienced Licensed Practical Nurse to join our outpatient team in a Federally Qualified Health Center setting. The LPN will support assessments, implement care plans, and provide patient education under supervision from the Practice Supervisor and RN.

This role emphasizes collaboration with RNs, physicians, MAs, and other health professionals to deliver high-quality care while maintaining patient privacy and safety standards.

Qualifications

• Apply clinical judgment to nursing decisions in outpatient care.
• Manage multiple tasks while delivering safe, high-quality care.
• Educate Medical Assistants on clinical tasks, patient care procedures, and safety practices.
• Demonstrate strong written and spoken communication for patient care notes.
• Ensure patient privacy and promote patient autonomy in care decisions.

Responsibilities

• Participate in data collection for nursing history, assessments, and social factors.
• Assist with physical examinations, vital signs, and targeted assessments.
• Monitor patient responses and report changes to RN or physician.
• Implement care plans and nursing interventions per established orders.
• Educate patients and families on health conditions and self-care strategies.
