Job summary

AdvantageCare Physicians (ACPNY) is seeking a practicing Internal or Family Medicine trained physician to join us in pursuit of high-quality patient care to provide a full scope of primary care services including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance.

Ideal candidates will have 2–3 years of outpatient primary care experience, the ability to diagnose and manage a wide range of chronic conditions, and a patient-centered approach with

Qualifications

• MD/DO or foreign equivalent.
• Three-year post graduate residency in Internal Medicine or Family Medicine.
• Board eligible or board certified in Internal Medicine or Family Medicine.

Responsibilities

• Review patient history and perform physical examinations, diagnose and treat, refer to specialists as needed.
• Provide quality clinical diagnostic skills and preventive health recommendations.
• Prescribe medications and formulate ongoing disease management plans.
• Provide relationship-centered care considering psychosocial and physical needs.
• Collect and maintain complete medical records.
• Communicate effectively with the clinical team to insure total delivery of quality care.
• Participate in medical staff educational programs and meetings.
