Qualifications

• Minimum 2–3 years of outpatient primary care experience.
• Board Eligible/Board Certified in Internal Medicine or Family Medicine.
• Active New York State license.
• Active DEA registration or eligible.
• Active Medicare/Medicaid enrollment or eligible.
• BLS or ACLS required.
• Strong communication skills, both verbal and written.

Responsibilities

• Review patient history and perform physical examinations; diagnose and treat; refer to specialists as needed.
• Provide quality clinical diagnostic skills and interpret test results to inform management.
• Prescribe medications and develop ongoing disease management plans.
• Deliver relationship-centered care considering psychosocial and physical needs.
• Maintain accurate and complete medical records and collaborate with the clinical team.
