CHRISTUS St. Vincent’s Center for Healthy Aging in Santa Fe, New Mexico, seeks a compassionate Nurse Practitioner for an outpatient practice. Provide comprehensive geriatric care, wellness visits, and management of chronic conditions in a team-based setting.

Qualifications

• Nurse Practitioner certification required
• Minimum of two years of Nurse Practitioner experience
• Active New Mexico Nurse Practitioner license, or ability to obtain
• Passion for caring for older adults and delivering patient-centered care

Responsabilités

• Conduct annual wellness visits and preventive health screenings
• Evaluate, diagnose, and treat acute and chronic conditions
• Manage common geriatric concerns: dementia, osteoarthritis, urinary incontinence, chronic diseases
• Order and interpret diagnostic tests and laboratory studies
• Develop and manage individualized treatment plans
• Collaborate with physicians, specialists, caregivers, and interdisciplinary team members
• Support patient goals while promoting independence, safety, and quality of life
