AlignMed Partners is seeking a full-time Advanced Practice Provider (NP or PA) for our Casa Real location in Santa Fe, NM. The role delivers primary and urgent care to nursing center residents, collaborating with the Center Leadership and Medical Director to meet patient needs while supporting our mission to improve health outcomes.

New grads welcomed; requires MSN NP with Older Adult/Family focus or ARC-PA‑accredited PA program; state licensure and DEA; two years geriatrics preferred.

Qualifications

• Graduate from an accredited School of Nursing, licensed as a registered nurse and certified as a Nurse Practitioner with MSN and Older Adult/Family NP track.
• Physician Assistant must graduate from ARC-PA accredited program and pass PANCE; state licensure and NCCPA certification required.
• APRNs with unencumbered licenses and DEA; PAs with unencumbered licenses and DEA for each practicing state.

Responsabilités

• Provide primary and urgent care services to nursing center residents on-site.
• Collaborate with Center Leadership and Medical Director to meet patient needs and AlignMed mission.
• Work with other providers to deliver high-quality health care and compassionate service.
