Job summary

CHRISTUS St. Vincent’s Center for Healthy Aging in Santa Fe, NM seeks a Geriatrician or Internal Medicine physician for a 100% outpatient practice. Flexible schedules and limited call, with an emphasis on independent, comprehensive care for older adults within a collaborative team.

The position offers Epic EMR, Nurse Navigation support, and a care model guided by 4Ms—Mentation, Medications, Mobility, and What Matters—ensuring patient-centered outcomes and quality of life.

Qualifications

• MD or DO required.
• Board certification in Geriatric Medicine or Internal Medicine required.
• Eligible for or currently hold NM medical license or license-eligibility.
• Previous outpatient or team-based care experience is valued.

Responsibilities

• Provide comprehensive outpatient medical care for adult and geriatric patients.
• Perform preventive health screenings and wellness evaluations.
• Diagnose and manage acute and chronic conditions common in older adults.
• Develop individualized treatment plans and coordinate with specialists and care team.
