Havenwyck Hospital Inc. in Auburn Hills, MI is seeking an Outpatient Clinician (Clinical Social Worker) to provide social services under the direction of the Director of Outpatient Services. You will be part of a multidisciplinary team delivering assessment, treatment planning, and discharge planning.

The role requires a Master of Social Work and an active LMSW or LPC license in Michigan, with a commitment to high-quality patient care and collaboration within a large health system.

Qualifications

• Master of Social Work degree required.
• Current License in the State of Michigan as a LMSW or LPC required.
• Employee must be in good standing to be considered for an internal transfer.

Responsibilities

• Participates as an integral member of a multidisciplinary treatment team.
• Engages in patient assessment, treatment planning, individual/family/group therapy, and discharge planning with proper documentation.
• Engages in supports coordination and collateral contact as needed.
• Works a schedule that meets program needs.
• Attends weekly treatment team meetings and milieu meetings.
• Participates in Performance Improvement activities.
