Havenwyck Hospital, an extension of Universal Health Services, is seeking an Outpatient Clinician (Clinical Social Worker) to provide social services under the direction of the Director of Outpatient Services. You will join a multidisciplinary treatment team and engage in comprehensive assessments, treatment planning, and discharge planning.

The role requires MSW and a current Michigan LMSW or LPC license, with a schedule that supports the program and weekly team meetings.

Qualifications

• Master of Social Work degree required.
• Current License in the State of Michigan as a Fully Licensed Social Worker (LMSW) or LPC required.
• Employee must be in good standing to be considered for an internal transfer.

Responsibilities

• Participates as an integral professional member of a multidisciplinary treatment team.
• Clinically documents patient assessment, treatment planning, individual therapy, family therapy, group therapy, and discharge planning.
• Engages in supports coordination and collateral contact as indicated by patient needs during programming.
• Works a schedule that meets the needs of the program.
• Attend weekly treatment team meetings.
• Attends program milieu meetings.
• Participates in Performance Improvement activities.
