Job summary
Reveleer is seeking a Clinical Documentation Specialist for outpatient CDI to improve documentation quality and RAF accuracy. You will review records, validate HCC diagnoses, and query providers to ensure complete documentation supporting risk adjustment.
The role relies on nursing expertise, proficiency with EMR systems (Epic/Athena/Cerner), and collaboration with care teams. This is a remote opportunity in the United States with competitive benefits.
Qualifications
- Graduate of an accredited nursing education program.
- Active and unrestricted RN or APRN license.
- Certifications: CDEO, CCDS-O, CDIP or CRC.
- Minimum of 2 years outpatient CDI experience.
- Experience navigating Epic, Athena, Cerner EMRs.
Responsibilities
- Review patient records to identify gaps in documentation and quality.
- Validate recapture and suspect conditions with clinical evidence.
- Query providers to ensure complete and accurate documentation.
- Create queries reflecting best practices for improvement.
- Recommend changes to clinical logic and workflows.
- Maintain patient confidentiality in line with privacy regulations.
