Job summary

UASI is seeking an experienced HCC Clinical Documentation Specialist for a fully remote role. You will support coding accuracy, HCC capture and documentation integrity across outpatient settings, applying strong clinical judgment and analytical skills.

The role emphasizes prospective chart reviews, reliance on Epic/OPAs, and collaboration to improve documentation completeness and severity reflection, with a focus on timely CDI interventions.

Qualifications

• Minimum 2 years ambulatory CDI or related documentation experience.
• Active RN or LPN license required.
• CCDS/CCD S-O/CDIP/CDEO/CRC certifications preferred.
• Proficiency with Epic workflows and OPAs preferred.
• Strong ICD-10-CM knowledge and HCC coding understanding.
• Excellent communication, critical thinking and organizational skills.

Responsibilities

• Serve as subject matter expert in clinical documentation quality, risk adjustment, and HCC integrity in outpatient settings.
• Perform prospective reviews to validate chronic conditions and ensure documentation accuracy.
• Utilize Epic and OPAs for chart reviews and identify documentation opportunities.
• Track work queues and ensure timely follow-up and CDI interventions.
• Monitor, analyze, and report CDI outcomes and HCC recapture performance.
