Qualifications

• Bachelor’s Degree in social services from an accredited college or university.
• Case Management experience highly desired.
• Knowledge of substance use disorders and treatment highly desired.
• Excellent interpersonal, written/verbal communication and computer skills.
• A valid SC driver’s license.

Responsibilities

• Frequent contact with various referral sources.
• Coordinating patient appointments.
• Sending text appointment reminders to patients.
• Auditing patient files for required documents.
• Helping to coordinate patient intake and counselor assignments.
• Providing referrals to external human service agencies.
• Managing non-clinical cases.
• Completing patient service authorizations.
• Ongoing communication with the Program Administrator and the clinical team.
• Completing documentation as necessary.
• Supporting Program Administrator in ensuring tasks associated with each case are complete.
