Job summary

SGMC Health Careers is seeking an experienced RN Outpatient Case Manager for the Main Campus, OUTPATIENT CASE MANAGEMENT department. This full-time, day-shift role (8:00 AM–4:30 PM) bridges hospital care and home care, providing education, referrals, and resources to support effective post-acute transitions.

The ideal candidate has at least 5 years as an RN, strong critical thinking, and experience with care coordination or case management.

Qualifications

• Graduation from an accredited school of nursing.
• Experience as an RN with at least 5 years.
• Preferred: care coordination, discharge planning, utilization review.
• Case Management certification may be preferred.
• Experience with Epic is a plus.

Responsibilities

• Coordinate care transitions and educate patients and caregivers.
• Guide patients and caregivers to resources post-hospitalization.
• Improve patient-physician communication during transitions.
• Address critical issues and self-management tasks without direct hands-on care.
