A behavioral health organization in Apache Junction seeks a Specialty Case Manager to provide comprehensive case management services to clients, focusing on justice-involved and high needs populations. This role includes developing individualized treatment plans, coordinating care, and maintaining ethical standards. The ideal candidate has at least one year of case management experience, particularly with the AHCCCS or SMI populations, and is proficient in electronic medical records. Qualifications
Minimum of one year experience with the AHCCCS population.
Experience with justice involved persons and chronically underhoused individuals.
Preferably one year experience with the Crisis/SMI population.
Responsibilities
Maintain strict adherence to ethical standards and legal requirements.
Complete case management functions focused on a housing-first model.
Develop individualized treatment plans and monitor treatment progress.
Collaborate with interdisciplinary teams to ensure client care.
Assist clients with AHCCS applications and other necessary paperwork.
Skills
Proficient in computer applications and electronic medical record systems
Proficiency with Microsoft Office Suite
Effective communication skills
Strong decision-making ability in emergencies
Knowledge of twelve step programs of recovery
Strong time management skills
Ability to type at least 45 wpm
Education
One year of case management knowledge and experience
Experience with the Crisis/SMI population
