Job summary

Pinnacle Peak Recovery in Scottsdale, AZ is seeking a Case Manager to oversee client caseloads from intake to discharge. You will coordinate ancillary services, document progress, and communicate with clinical leadership to ensure quality care.

The role requires addiction counseling credentials and a strong adherence to confidentiality and regulatory guidelines. Ideal candidates bring two years of experience in the addiction field, CPR/First Aid/HIPAA training, and the ability to prioritize

Qualifications

  • Must have two years clean and sober if in recovery.
  • Licensed or credentialed in chemical dependency field (RAS/CAS/CADC/CATS/CDC/CAADAC/NAADAC or equivalent).
  • Minimum two years experience in addiction field.
  • Ability to prioritize and provide appropriate treatment protocols.
  • Maintain current certifications for this position.

Responsibilities

  • Manage clients from intake to discharge and track progress.
  • Coordinate ongoing and ancillary care including legal, vocational, insurance referrals.
  • Document all client activities in the client chart.
  • Attend mandatory weekly staff meetings and follow program protocol.
  • Maintain compliance with federal, state, and confidentiality regulations.

Case Manager

Manage clients' cases from intake to discharge, Follow clients' progress, keep Clinical Director informed on any issues/problems with any clients. The Case Manager is charged with coordinating on-going and ancillary care for the client that may include but is not limited to legal appointments, vocational issues, insurance coordination, external referrals for ancillary care. The Case Manager will document all of the client activities that s/he is responsible for in the client chart.

Duties and Responsibilities:
  1. 1. The Case Manager may maintain a caseload and be involved in client treatment processes: a. Responsible for conducting group process. b. Maintains treatment plans, master problems list, progress notes and all other required client documents pertaining to treatment. c. Maintains contact with client's family on a weekly basis discussing progress/problems depending on client's release of information. d. Assures proper client consents and that other treatment documents are completed e. Maintains contact with client on a daily basis, provides one-on-one sessions with clients two to three times a week if necessary to coordinate services. f. Responsible for charting one-on-one sessions, groups, and any and all incidents, accidents, and pertinent information relating to client care. g. Meets with client on an individual basis when necessary. h. Corresponds with client's probation officer and the courts if applicable. Maintains contact with Clinical Director and Program Director regarding caseloads. j. Establishes individual assessment and treatment program. k. Insures clients attend and participate in all required groups and complete all related assignments
  2. 2. Responsible for attending and participating in mandatory weekly staff meetings to discuss client care and all aspects of administration.
  3. m. Knowledge of policy and procedure manual relating to program protocol.
  4. n. Other duties as assigned.
  5. 3. Case Manager performs and maintains professional abilities in the following areas: a. Maintain and adhere to all Federal, State, and County regulations regarding client confidentiality and staff code of ethics. b. Perform tasks the supervisor assigns including, but not limited to, schedule, case load, and deadlines. c. Continuous training in areas where needed, attendance at seminars, continue earning CEUs. d. Treatment planning is kept current and fits with the needs of the client. e. Crisis intervention—recognizing emotional stress and utilizing resources to resolve problems. f. Client education—provide current alcohol/drug information and available resources to clients, groups, and families.
