Manage clients' cases from intake to discharge, Follow clients' progress, keep Clinical Director informed on any issues/problems with any clients. The Case Manager is charged with coordinating on-going and ancillary care for the client that may include but is not limited to legal appointments, vocational issues, insurance coordination, external referrals for ancillary care. The Case Manager will document all of the client activities that s/he is responsible for in the client chart.
