Qualifications

Responsibilities

• Coordinate care during patient visits: rooming, vitals, history, updating medications, and documenting in EHR; schedule follow-ups as needed.
• Provide patient education on health management and care coordination for preventive and specialty visits.
• Perform nursing skills within scope (injections, vaccinations, labs, x-rays) as ordered by the provider.
