Horizon Health Care, Inc in Woonsocket, SD is seeking a full-time Registered Nurse, Licensed Practical Nurse, or Certified Medical Assistant to provide nursing care in an outpatient clinic setting. The duties will be split between front office and nursing care, with emphasis on high-quality, patient-centered care.

Qualified candidates must possess a valid South Dakota nursing license and CMA certification for Medical Assistants.

Qualifications

• Must hold a valid SD Nursing License.
• Medical Assistants must be certified through an accredited program.
• Experience in outpatient care and patient education is preferred.

Responsibilities

• Coordinate care during patient visits, including rooming, vitals, history, and updating medications in the EHR.
• Educate patients on health management and coordinate referrals for preventive and specialty care.
• Perform nursing skills such as injections, vaccinations, labs, and imaging per license scope.
• Manage appointment scheduling to align with provider availability and visit lengths.
• Collect and verify demographic and insurance information with confidentiality.
