Qualifications

• Certification as a Medical Assistant or Care Coordination certification.

Responsibilities

• Coordinate outpatient care management activities and ensure timely access to healthcare and community resources.
• Support transitions of care and promote patient engagement and adherence to care plans.
• Collaborate with providers, nurses, social workers, case managers, and interdisciplinary teams to improve patient outcomes and reduce avoidable hospital utilization.
