Job summary

Northwestern Medicine is seeking an Ambulatory Nurse Care Coordinator to connect patients with the right care pathways and support teams. The role centers on education, care management coordination, and collaboration with physicians and staff to improve outcomes in an ambulatory setting.

You will review patient data, identify barriers to care, and implement education plans while coordinating post-discharge follow-up and referrals.

Qualifications

• Current license as a Registered Nurse in the State of Illinois (RN).
• BSN degree with three to five years of relevant clinical experience or complete BSN within two years of hire.
• 3 years of experience (5 years preferred) in care coordination, chronic conditions, and health outcomes.
• Willingness to travel may be required.

Responsibilities

• Serve as patient advocate in ambulatory settings and assist navigation across care settings.
• Address care coordination needs for high to moderate risk patients and assist with new patients.
• Review records and data to identify patients needing care coordination and assess barriers to care.
• Consult with multidisciplinary team to develop, implement, and evaluate care plans.
• Provide triage and education to patients and families about processes, treatments, and health management.
• Ensure continuity of care post-discharge, coordinate referrals, and manage transitions of care.
