Catholic Health is seeking an experienced Nurse Practitioner or Physician Assistant to join the outpatient Internal Medicine and Cardiology team at CIMA in Massapequa, NY. The APP will provide comprehensive care to adults, emphasizing preventive medicine and cardiovascular health in a collaborative setting.

Qualificações

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Master's degree in Nursing or Physician Assistant Studies or equivalent.
• Current, unrestricted New York State NP/PA license.
• Current national board certification appropriate to the provider.
• Current BLS and ACLS certifications.
• Strong clinical assessment, diagnostic, and communication skills.
• Ability to work independently within the authorized scope of practice while collaborating with physicians and other team members.
• Commitment to delivering compassionate, patient-centered care.

Responsabilidades

• Conduct comprehensive patient assessments, including histories, physicals, and evaluation of acute and chronic conditions.
• Diagnose and manage hypertension, diabetes, hyperlipidemia, and cardiovascular disease.
• Develop and implement individualized treatment plans with supervising physicians.
• Order, review, and interpret labs, EKGs, imaging, and other studies.
• Assist with cardiac diagnostics and stress testing when indicated.
• Prescribe medications per NYS regulations and privileges.
• Provide routine follow-up and adjust care plans as needed.
• Educate patients and families on disease prevention, nutrition, exercise, and lifestyle changes.
• Coordinate referrals to specialists and ensure continuity of care.
• Collaborate with cardiologists, internists, nurses, and staff; maintain EMR documentation.
• Participate in quality improvement initiatives and adhere to clinical standards.
