Zusammenfassung

Catholic Health's CIMA Medical Practice in Massapequa, NY is seeking an Advanced Practice Provider (NP/PA) to join our outpatient Internal Medicine and Cardiology team. The APP will work closely with physicians and staff to deliver comprehensive, preventive, and cardiovascular care to adults.

The position involves travel to East Hills and West Babylon locations once per week, and it is a full-time outpatient role with responsibilities spanning assessments, orders, treatment planning, and care

Qualifikationen

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Master's degree in Nursing or Physician Assistant Studies, or equivalent.
• Current, unrestricted New York NP or PA license.
• Current national board certification for the provider type.
• Current BLS and ACLS certifications.

Aufgaben

• Conduct comprehensive patient assessments and histories.
• Diagnose and manage hypertension, diabetes, lipids, and cardiovascular disease.
• Develop and implement individualized treatment plans with physicians.
• Order and interpret labs, EKGs, imaging, and other studies.
• Assist with cardiac diagnostics and stress testing when indicated.
• Prescribe medications following New York State regulations.
• Provide routine follow-up, monitor effectiveness, and adjust plans.
• Educate patients on prevention, nutrition, exercise, and lifestyle.
• Coordinate referrals and ensure continuity of care.
• Collaborate with cardiologists, internists, nurses, and staff.
• Maintain accurate EMR documentation and contribute to quality initiatives.
