Catholic Health is seeking a compassionate Advanced Practice Provider (NP/PA) to join the outpatient Internal Medicine and Cardiology team at CIMA in Massapequa, NY. The APP will work with cardiology and internist physicians to provide comprehensive adult care with a focus on prevention and chronic disease management.

The role requires strong clinical assessment, ability to manage acute and chronic conditions, and collaboration within a multispecialty practice.

Qualifications

• Graduate of an accredited NP/PA program.
• Master's degree in Nursing or Physician Assistant Studies, or equivalent credential.
• Current NY NP/PA license and national certification.
• BLS and ACLS certifications.

Responsibilities

• Conduct comprehensive patient assessments and histories.
• Diagnose and manage hypertension, diabetes, hyperlipidemia, and cardiovascular disease.
• Develop individualized treatment plans with supervising physicians.
• Order and interpret labs, EKGs, imaging as appropriate.

Travel Requirements: This position is primarily based in Massapequa, with required travel to our East Hills location one day per week and our West Babylon location one day per week.

Dr. Greg R. Blair is an experienced cardiologist practicing in Massapequa, New York, with more than 20 years of experience in cardiovascular medicine. He earned his medical degree from SUNY Upstate Medical University and is affiliated with St. Francis Hospital & Heart Center and St. Catherine of Siena Hospital, both part of Catholic Health. Dr. Blair specializes in the diagnosis and management of cardiovascular conditions, including coronary artery disease, hypertension, and cardiac arrhythmias. He is dedicated to providing comprehensive, patient-centered cardiac care, with an emphasis on preventive medicine, early detection, and long-term cardiovascular health.

Position Summary

Catholic Health is seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient Internal Medicine and Cardiology team at CIMA in Massapequa.

The Advanced Practice Provider (APP) will work closely with our physicians and clinical staff to provide comprehensive care to adult patients, with an emphasis on preventive medicine, chronic disease management, and cardiovascular health.

The ideal candidate will have strong clinical assessment skills, experience managing acute and chronic medical conditions, and an interest in providing long-term, patient-centered care within a collaborative multispecialty practice.

Key Responsibilities

• Conduct comprehensive patient assessments, including medical histories, physical examinations, and evaluation of acute and chronic medical conditions.
• Diagnose and manage common medical conditions, including hypertension, diabetes, hyperlipidemia, and cardiovascular disease.
• Develop and implement individualized treatment plans in collaboration with supervising or collaborating physicians, as applicable.
• Order, review, and interpret laboratory tests, EKGs, diagnostic imaging, and other appropriate studies within the provider's scope of practice.
• Assist with cardiac diagnostic evaluations and coordinate additional testing, including stress testing, when clinically indicated.
• Prescribe medications and recommend therapeutic interventions in accordance with applicable New York State regulations and clinical privileges.
• Provide routine follow-up care, monitor treatment effectiveness, and adjust care plans as appropriate.
• Educate patients and families regarding disease prevention, medication management, nutrition, exercise, and lifestyle modifications.
• Coordinate referrals to specialists and facilitate continuity of care across the practice.
• Collaborate with cardiologists, internists, nurses, and administrative staff to ensure efficient, high-quality patient care.
• Maintain accurate and timely documentation in the electronic medical record.
• Participate in quality improvement initiatives and support established clinical standards and protocols.

Qualifications

Required

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Master's degree in Nursing or Physician Assistant Studies, or an equivalent qualifying PA educational credential.
• Current, unrestricted New York State Nurse Practitioner or Physician Assistant license.
• Current national board certification appropriate to the provider's professional designation.
• Current BLS and ACLS certifications.
• Strong clinical assessment, diagnostic, and communication skills.
• Ability to work independently within the authorized scope of practice while collaborating effectively with physicians and other clinical team members.
• Commitment to delivering compassionate, patient-centered care.

Preferred

• Experience managing adult patients with chronic medical conditions.
• Familiarity with cardiovascular disease management and interpretation of EKGs.
• Previous experience in a busy outpatient medical practice.
• Proficiency with electronic medical records.
