Job summary

Catholic Health in East Hills, NY is seeking a compassionate Advanced Practice Provider (NP/PA) to join a six-physician internal medicine group. The APP will deliver patient‑centered primary care to adults, collaborate closely with physicians, and manage chronic diseases in a supportive team setting.

This M-F role includes a Saturday rotation, with emphasis on preventive care, wellness visits, and comprehensive disease management in a well-established practice that serves the local community.

Qualifications

• Licensed Nurse Practitioner in the State of New York (or eligible).
• National board certification required.
• BLS/ACLS/DEA required.

Responsibilities

• Provide comprehensive primary care services to adult patients under physician collaboration.
• Evaluate, diagnose, and manage acute and chronic medical conditions.
• Manage chronic disease states including diabetes, hypertension, obesity, and cardiovascular risk factors.
• Perform preventive care visits, annual wellness exams, and Medicare wellness visits.
• Order and interpret diagnostic tests and labs.
• Prescribe medications in accordance with New York State regulations.
• Educate patients on disease management, lifestyle modification, and preventive health.
• Document clinical encounters accurately and efficiently in the electronic medical record (EMR).
• Coordinate care with specialists, hospitals, and ancillary services as needed.
• Support quality initiatives and evidence-based care standards.
