Job summary

Duke Health invites applications for a Behavioral Health Therapist at Signature Care of South Durham. The role collaborates with physicians and a multidisciplinary team to provide holistic assessments and treatment planning for adults within a Primary Care Medical Home.

The position requires a Master’s in a related field and current licensure as LCSW/LMFT/LCMHC; LCAS is preferred. Schedule is day shifts, Monday–Friday, with no weekends.

Qualifications

• Master's degree in human services or related field.
• Current licensure as a licensed LCSW, LMFT, and/or LCMHC; LCAS preferred.

Responsibilities

• Perform psychosocial assessments and develop comprehensive treatment plans.
• Document assessments, plans, interactions, and interventions in the electronic health record per guidelines.
• Provide education to patients and families and coordinate with the interdisciplinary team to improve care and workflow.
