Job summary

Lexington Health is seeking an Outcomes Coordinator in Quality Management - Acute to guide performance improvement efforts at Lexington Medical Center in West Columbia, SC. You will coordinate data collection and analysis, ensuring data integrity and regulatory compliance with DNV and SC DHEC standards.

Requires a BSN, SC RN license, and 5 years RN experience including 2 years in utilization review, quality assurance, or case management plus 2 years in a leadership role.

Qualifications

• Bachelor of Science in Nursing required.
• ADN to MSN pathway may be considered if BSN curriculum completed and MSN completed within 12 months.
• Licensed RN by the South Carolina State Board of Nursing.

Responsibilities

• Coordinate data collection and analysis for performance improvement activities.
• Develop and revise programs, protocols, and guidelines for care of identified patient populations.
• Provide educational offerings to support professional development and comply with regulatory requirements.
• Perform statistical analysis, summarize findings, and prepare reports for stakeholders.
• Lead CQI teams using established tools and methods.
