Job summary

Qualifications

Responsibilities

• Create, implement, and evaluate care plans; adjust as needed to achieve outcomes.
• Educate patients, families, nurses, and community members on health matters.
• Delegate tasks and coordinate with the care team to ensure patient safety and quality care.
• Document learning needs and ensure timely follow-up and communication with the team.
• Support peers and participate in professional development and community service.
