Evolve Therapy Services in Butler, AL is seeking a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility on a PRN basis. You will plan, organize, and direct OT services and collaborate with a dedicated team to ensure high-quality resident care.

You will screen residents, implement treatment plans under supervision, and maintain precise documentation aiming to CMS standards. This role emphasizes teamwork, patient-centered care, and professional growth.

Qualifications

• An associate degree in OT is required.
• MUST hold active OTA licensure in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to identify best OTA interventions.
• Implement treatment plans under supervision and track goals.
• Communicate evaluation findings and plans to residents/families and the interdisciplinary team.
• Coordinate therapy schedules with team members.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform other duties to support resident care.
• Comply with CMS documentation standards daily.
