Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Butler, AL. This PRN role involves evaluating residents, planning and directing OT services, and coordinating with an interdisciplinary team to deliver high-quality care.

Qualifications

• Must hold a Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine interventions.
• Evaluate residents to develop treatment plans with goals.
• Communicate findings and plans to residents, families, and team.
• Establish resident therapy schedules with team.
• Deliver OT services per plan and document/bill accordingly.
• Perform other duties to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)
