Evolve Therapy Services seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Butler, Alabama. This PRN role focuses on delivering individualized occupational therapy services within a team that includes supervising therapists and administrators.

The OTA will screen, assess, and implement treatment plans, ensuring documentation and billing align with CMS standards while pursuing ongoing licensure and professional development.

Qualifications

• Screens residents to determine best intervention per scope of practice.
• Implements treatment plans with short- and long-term goals under supervision.
• Communicates evaluation findings and plans to residents, families, and team.
• Establishes resident therapy schedules with team members.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment per scope of practice.
• Perform treatment plans for residents; follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, plans, and recommendations to residents, families, and team.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to plan and document/bill services.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards daily with on-site documentation.
