Paradigm at Faith Memorial in Pasadena, TX is seeking a Licensed Occupational Therapist to join our in-house team serving residents with person-centered care.

You will design therapy plans, help residents regain independence in daily activities, collaborate with nurses and physicians, and educate residents and families to extend progress beyond sessions. New grads welcome with mentorship and a supportive team.

Qualifications

• Licensed Occupational Therapist in good standing.
• SNF or post-acute experience preferred, but new grads welcome with mentorship.
• Excellent communication and documentation abilities.

Responsibilities

• Design personalized therapy plans reflecting residents' goals.
• Help residents regain independence in daily activities and cognition.
• Collaborate with nurses, PTs, SLPs, and physicians on interdisciplinary team.
• Educate residents and families to sustain progress beyond therapy sessions.
• Maintain thorough progress documentation and quality care.
