Job summary

Stern Therapy Consultants is seeking a PRN Occupational Therapist to join The Elms Center in Milford, NH. You will apply clinical skills to help skilled nursing residents regain daily routines, adapt to changing abilities, and work toward meaningful rehabilitation goals.

Qualifications

• Active New Hampshire Occupational Therapist license.
• Sound clinical judgment and strong treatment planning skills.
• Clear communication and accurate, timely documentation.
• Experience in skilled nursing or post-acute settings is a plus; new graduates welcome to apply.

Stern Therapy Consultants is looking for a PRN Occupational Therapist at The Elms Center in Milford, NH. Use your clinical skills to help skilled nursing residents rebuild daily routines, adjust to changing abilities, and work toward meaningful goals during rehabilitation.

What You’ll Do

• Assess how strength, coordination, cognition, and endurance affect residents' ability to manage everyday tasks.
• Establish individualized plans of care with measurable goals for self-care and functional participation.
• Deliver treatment through practical activities such as dressing, grooming, bathing, and toileting.
• Identify adaptive equipment and environmental modifications that support safer task performance.
• Educate residents and caregivers on strategies they can use between sessions and after discharge.
• Review progress, oversee COTA services within the treatment plan, and communicate recommendations through documentation and care team discussions.
