Continuum Therapy Partners seeks a PRN Occupational Therapist for Little Falls Care Center in Little Falls, MN. The role ensures continuity of care with the same clinical standards and voice across shifts.

The ideal candidate holds a Master’s or Doctorate in Occupational Therapy, an active Minnesota OT license, NBCOT certification, and about 1 year of experience. You will evaluate patients, develop treatment plans, document progress, and educate families on home safety and adaptive equipment.

• Active state license in Minnesota; NBCOT certification required.
• 1 year of OT experience preferred.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required


• Current, unencumbered state Occupational Therapy License; NBCOT Certification required


• 1 year of previous experience as an Occupational Therapist preferred


• Evaluates and develops individualized treatment plans to improve patients' functional independence


• Documents patient progress and modifies treatment plans based on clinical outcomes


• Educates patients and families on adaptive equipment and home safety recommendations
