Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure excellent patient outcomes.

You will also supervise OT Assistants, document diligently, and communicate progress and expectations to patients and families. Join a dynamic team dedicated to high-quality rehabilitation care.

Qualifications

• Assess patients to determine functional status and goals.
• Develop and adjust individualized plans of care.
• Communicate clearly with patients to explain diagnoses and progress.
• Collaborate with nursing and facility staff to ensure patient-centered care.

Responsibilities

• Provide high-quality OT services to patients in SNF settings.
• Deliver excellent customer service to patients and families.
• Collaborate with facility staff to ensure optimal patient outcomes.
• Evaluate new patients and set measurable functional goals.
• Treat patients, educate them on injuries, progress and outcomes.
• Re-evaluate progress and revise plans to reset goals.
• Maintain timely and accurate documentation of evaluations, re-evaluations, discharges and daily notes.
• Supervise Occupational Therapy Assistants.
