• Experience with ADL and cognitive rehab a plus

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments
• Create and implement personalized care plans
• Collaborate with team to ensure quality outcomes
• Maintain documentation to meet regulatory requirements
• Obtain and update physician orders
